Must be completed by EACH CVIM student. Complete form only once, even if you are in multiple music classes. Siblings, please complete your own form. This data is gathered through a Google Form and is used to generate Field Trip Waiver, Student Insurance Affidavit and Medical Authorization. It is for ALL music students. NOTE: You only need to update it ONCE per year (unless your information changes), even if you are in multiple music classes. THANK YOU!
